Lantern Scholar Nomination

This form is for counselors and CBO partners only. If you are a student and would like to apply for the Lantern Scholars program, please use our student application form.

Lantern scholars will participate in an overnight program on the following dates:

Saturday, October 5–Monday, October 7

Students must be available for the dates of the event to participate in this program.

Please review this form before starting the nomination. You must answer all required questions and submit all documents at once. This form cannot be saved and finished at a later time.
